• This paper was given to me in September '07 by the Alaska Salvation Army H.R. Director, when I asked for the papers used to take away my medical benefits. 
• The xxx-0223 telephone number did not exist on March 21. The number was issued to me by the phone company in April. The first bill is here.  

• I did not live at the address below yet on March 21. 

• The form was created over a change-of-address form I turned in sometime in April or May.

• It was common to falsify paper to expel or punish a client, but this is one of the times Salvation Army Directors did it to staff.
